Registration forms have information about the club, dates for club meetings, and registration fees where it applies.
Lower School Clubs
K-2 STEM Club This club meets on Thursdays, 3-4:15. The first meeting is September 18th. Register as soon as possible. Registration is limited.
3-4 STEM Club This club meets on Wednesdays, 3-4:15. The first meeting is September 24th. Register as soon as possible. Registration is limited.
Middle School Clubs
NEW (added on Thursday, 10/23/2025) Musical Theater Showcase
Grades 5-8
Dates: Thursday, November 13th – Thursday, February 5. Performances February 5 and 6
Time: Thursday, 3pm-5pm
Where: Music Room
Deadline to Register: Turn in the form by Wednesday, November 5. The club limit is 20. First come, first serve. Don’t wait to register.
Theater Appreciation Outing – Registration Form. Theater Appreciation Outing – WAIVER.
Grades 5-8.
When: Sunday afternoon on October 26!
Deadline: Turn in the form AND waiver by Wednesday, October 15th. Turn the forms into Hilary at hilarypr@fsmn.org.
Important Note: Our first meeting with students will be from 3:00-3:30 pm on Thursday, October 23 – a few days before the show. In that half hour, we’ll preview the plan, the play itself, and expectations for the day of the outing.
Mock Trial Fall 2025. Grades 5-8. This club meets Wednesdays, 3-4:30pm. The first meeting is Wednesday, October 29th. Questions about how mock trial works? Email Chrissy at chrise@fsmn.org.
Environmental Action Club (EAC) Registration Form Grades 5-8. This club meets during the school day.
Outdoor Club* Grades 5-8. This club meets on weekends and Friday nights. Note: there is school provided transportation to some of the events and other events require that the family provides transportation. The form lists Sept. 5th as the registration deadline. We will accept registration until this Friday, September 12th (the first event is Saturday, September 13th)
*Outdoor Club Waiver Form. If your student is participating in outdoor club, they must have this waiver completed and turned in before the first event.
ZOOMS STEM Design Challenge Club Grades 5-8 – This club meets on Mondays, 3-4:30. The first meeting is October 13th. Register is due by October 8th. Registration is limited to 12 participants.

- Return the registration form to the front office by emailing it to frontoffice@fsmn.org or dropping off the hard copy* registration form to Travis in the front office.
*If you need a hard copy registration form, ask Travis in the front office.
