Extended Day Contract Change Form
If you already have an extended day contract and you want to CHANGE your days/times, use this Extended Day Contract Change Form to change your extended day contract.
- Return the above Extended Day Contract Change Form to the front office by noon on the last school day of the month.
- All contract changes will take effect on the 1st of the following month.
- All billing changes go into effect the 1st of the following month.
